Thank you for applying for a transitional housing unit at Path Haven.

Youth may fill out the Application for Occupancy below. Financial assistance for rentals is available. Provide your contact information. Select the Supervised Independent Living (SIL) Program for participation while in occupancy at Path Haven. The two SIL programs include the College Bound Program and the Employment Independency Program. Upload copies of your most recent resume and transcript. Print your name to sign, then submit the application. Your application will be submitted for review. Youth may be required to submit additional information for applications. Upon completion of review, youth will receive an email from the Program Manager regarding application status.

Applicants must first be fully eligible by the Tennessee Department of Children Services (DCS) and Path Haven for occupancy.
