Apply for Transitional Housing at Path Haven
Thank you for applying for new transitional housing unit at Path Haven.
Fill out the Application for Residency below. Provide your contact information. Select the Supervised Independent Living (SIL) Program for participation while residing at Path Haven. Upload a copy of your most recent resume and transcript. Print your name to sign and complete the application. Your application will be submitted for review. You may be required to submit additional information to your application. Upon completion of review, you will receive an email from the Program Manager or another staff member regarding the status of your application.
